Nervous System Regulation: What It Actually Means
Kelowna counsellor Lindsey McDonald on what the phrase really describes, what a stuck system feels like, which practices have evidence behind them, and what is being oversold.

You’ve probably seen the phrase four times this week. Someone on your feed is humming into their chest. Someone else is selling a supplement for it. A friend mentioned her therapist said it and now you’re wondering whether the thing your body does at 3am has a name.
It does, roughly. The language is useful and it’s also being stretched well past what the research supports, which makes it hard to tell what’s worth your attention.
Nervous system regulation refers to your body’s ability to shift into a stress response when a situation demands it, and to settle back down once the situation has passed. The word describes flexibility rather than constant calm. A body that can get activated and then recover is doing exactly what it’s built to do.
What does nervous system regulation actually mean?
Regulation means your body can rev up and then come back down. Stress arrives, your system responds, the situation resolves, and your heart rate, breathing and muscle tension return to baseline. The recovery is the part that matters. Getting activated is not the problem, and it never was.
The autonomic nervous system runs this without asking your permission. It manages heart rate, breathing, digestion, pupil size and dozens of other things you don’t choose. One branch mobilises energy, the other conserves and restores it. Both are supposed to take turns throughout the day.
Where the popular version goes wrong is in treating calm as the goal. People arrive in my office believing that a regulated person feels peaceful most of the time, and that any surge of anger or fear means something has gone wrong with them. A regulated body still gets angry in traffic. It just doesn’t still be angry three hours later.
The reason you’re seeing the phrase everywhere is partly cultural. The Global Wellness Summit named neurowellness one of the defining trends of 2026, and where a trend goes, products follow.
What are the three states people talk about?
Most descriptions sort autonomic experience into three broad states: activated, shut down, and settled. The categories are simplifications of something far messier, and they give people useful words for sensations that previously had none. Recognising which one you’re in is often the first practical step.
Heart rate up, muscles ready, attention narrowed. Useful for a deadline or a near miss on the highway. Costly when it never switches off.
Heavy, foggy, far away. The system pulls energy back when activation stops working. Often mistaken for laziness or low mood.
Enough arousal to engage, not so much that you cannot think. Conversation feels possible. Food digests. Sleep arrives.
Most days move between all three. You wake settled, get activated by an email, drop into fog by mid-afternoon, and come back to yourself over dinner. Trouble shows up when one state becomes home and the others become visits.
A note on the theory behind these categories, since you’ll meet it if you read further. This three-state map is usually attributed to polyvagal theory, and that theory is under serious dispute. In 2025 a large group of physiologists published a formal evaluation in Clinical Neuropsychiatry arguing that its evolutionary and physiological premises do not hold up.
I still use the vocabulary, carefully, because clients find it clarifying to have names for these states. What the critique changes is how much weight the underlying mechanism can bear. The observations survive independently: slow breathing measurably shifts heart rate variability, social contact reduces physiological arousal, and movement changes how activation resolves. Those findings don’t depend on the theory being right about vagus nerve evolution.
What does a dysregulated nervous system feel like?
Dysregulation usually shows up as a body that won’t come down or won’t come up. People describe being exhausted and unable to sleep, or flat and unable to feel much of anything. The common thread is a stress response that has stopped completing its cycle, so the reaction outlasts whatever set it off.
- Tired all day, alert the moment your head hits the pillow
- Jaw, neck or shoulders that never quite let go
- Startling at the toaster, the phone, a car door
- Digestion that misbehaves with no medical explanation
- Swinging between doing too much and doing nothing
- Feeling far away from your own life, as though watching it
- Small setbacks landing like large ones
Two cautions about that list. First, every item on it also appears in thyroid conditions, anaemia, sleep apnea, perimenopause and a dozen other things worth ruling out with your doctor. I’ve had clients spend months on breathing practices for what turned out to be a B12 deficiency.
Second, dysregulation describes a pattern rather than naming a diagnosis. You will not find it in the DSM. That doesn’t make the experience less real, and it does mean the term can be stretched to explain almost anything, which is precisely what makes it good marketing. When someone tells you your bloating, your breakup and your procrastination all come from the same stored trauma, treat that with the scepticism you’d give any other single-cause explanation.
The overlap with high-functioning anxiety is substantial. So is the overlap with grief, which does its own strange things to sleep, appetite and energy.
Why does my body stay stuck in one state?
Bodies get stuck when activation keeps arriving faster than it resolves. Chronic stress, unresolved loss, illness, poor sleep and long stretches of vigilance all keep the system braced. The threshold for what counts as threatening drops, so smaller things set off larger responses.
Daniel Siegel described this as the window of tolerance: the range of arousal within which you can think clearly, feel your feelings and stay connected to the person in front of you. Push above it and you get overwhelm. Drop below it and you get numbness. Long-term stress narrows the window, so the same Tuesday that used to be manageable now isn’t.
Things I see narrow that window most reliably:
- Sleep debt. Nothing else on this list works properly when sleep isn’t happening, and the relationship runs both directions.
- Ongoing situations without an ending. Caregiving, a health condition without answers, a workplace you cannot leave yet. The body is poor at resolving stress it cannot escape.
- Grief, particularly the kind nobody acknowledges. Unwitnessed loss keeps the system working overtime.
- Chronic illness. Pain and fatigue are themselves ongoing stressors, and the loop between them and arousal is well documented.
- Constantly monitoring other people. If you grew up reading the room for safety, your system may still be scanning, which sits close to people-pleasing.
None of this means your body is broken. A system that stays braced in conditions that genuinely called for bracing is working correctly. The work is teaching it that the conditions have changed.
How do you regulate your nervous system?
The practices with the strongest evidence are unglamorous: slow breathing with a long exhale, regular movement, protected sleep, and time with people whose presence steadies you. Most work by repetition rather than by intensity. Doing something small on ordinary days builds more capacity than doing something dramatic during a crisis.
Slow breathing with a longer exhale. This has the best evidence of anything on the list. A systematic review and meta-analysis of voluntary slow breathing found consistent increases in vagally-mediated heart rate variability during practice, immediately after a single session, and after multi-session training. Roughly six breaths per minute, with the out-breath longer than the in-breath. Five minutes is plenty to start.
Orienting. Let your eyes move slowly around the room and land on whatever they want to land on. Turn your head rather than just your eyes. It sounds too simple to matter, and it’s one of the first things I do with clients who arrive activated, because it gives the body current information instead of remembered information.
Movement that finishes. Walking, shaking out your hands, carrying something heavy up a hill. Activation is energy that was mobilised for action, and giving it an action to complete tends to resolve it faster than sitting still and waiting.
Co-regulation. Nervous systems settle in the presence of other settled nervous systems. This is not a metaphor, it’s measurable in heart rate and skin conductance, and it’s a large part of why counselling works at all. Sitting with a friend who isn’t alarmed by you does something that no breathing app replicates.
Sleep, light and food. Unfashionable, and load-bearing. Morning daylight, a consistent wake time, and eating before you’re desperate will do more for your baseline than any technique in this article.
A word on cold water, since it comes up constantly in Kelowna where the lake is right there. Cold exposure produces a real and sharp physiological response, and some people find it genuinely helpful for interrupting a spiral. For others it’s another jolt to a system already receiving plenty of jolts. Notice which one you are.
What’s being oversold about regulating your nervous system?
The oversold version promises that one technique, device or supplement will reset your system permanently. Regulation is a capacity you build through repetition, closer to fitness than to a switch. Anything marketed as a one-time fix for a lifetime pattern is selling you the wrong shape of thing.
Specific claims worth pausing on:
- Devices and supplements that promise to “reset the vagus nerve.” The research behind consumer versions of these is early, small, and mostly funded by the people selling them.
- Any framework that explains every symptom you have with one mechanism. Real bodies are less tidy.
- Trauma release claims that go well past the evidence. Somatic work has promising research behind it and the trials so far have been small, which is a reasonable thing for a practitioner to say out loud.
- Regulation used as a way to avoid feeling. This is the one I watch for most in sessions.
That last point deserves more than a bullet. There’s a version of this work where breathing exercises become a way to shut down anger before you have felt it, or to make grief quieter so it stops inconveniencing anyone. That looks like regulation from the outside. The useful question afterward is whether you feel more present in your life, or further away from it.
When does counselling help with this?
Counselling helps when self-directed practice keeps hitting the same wall, when the pattern connects to something that needs a witness, or when your window has narrowed enough that daily life has shrunk to fit it. The relationship itself does part of the work, because co-regulation needs another person and you cannot supply one on your own.
I have training in Somatic Experiencing, which means sessions often include attention to what your body is doing while we talk, not only to the content of what happened. That might be noticing where your breath stops when you mention a particular person, or staying with a small sensation long enough for it to change on its own. We go slowly, and you stay in charge of the pace.
For some people talking is the right tool and the body work is a small part of it. For others, and I see this often with clients who can explain their patterns perfectly and still feel stuck in them, the body-based work is where things finally shift. The honest timeline: most clients notice they recover from stress a bit faster before they notice being stressed less often. That earlier recovery is the measure worth watching.

Can you regulate your nervous system on your own?
Partly. Breathing practices, movement, sleep, and time outdoors all help, and you can do them without a therapist. What is harder to do alone is co-regulation, the settling that happens in the presence of a calm other person, and the slower work of processing whatever keeps setting the alarm off.
What is the difference between regulation and suppression?
Regulation lets a feeling move through you and finish. Suppression stops the feeling from surfacing at all. Breathing exercises used to shut down anger or grief before you have felt it will look calm on the outside while the pressure stays. The test is whether you feel more present afterward, or further away.
Does somatic therapy help with nervous system regulation?
The clinical evidence base is promising and still small. A randomized controlled trial found brief Somatic Experiencing reduced PTSD symptoms in people with chronic low back pain, and most other studies have been small with varied methods. In practice, body-based work suits people who understand their patterns intellectually but stay stuck physically.
Is nervous system dysregulation a diagnosis?

How is this different from anxiety, and do I need a different kind of counselling?
They overlap heavily. Anxiety describes the thoughts and the felt experience, while regulation language describes the physiology underneath. Most people benefit from work that addresses both, which is why sessions often move between talking about what happened and noticing what the body does while you talk about it.
If your body has been braced for a long time, that bracing made sense when it started. It can also be unlearned, slowly, with repetition and usually with company.
